"Parent - I wish my son/daughter to be a player member of Heyford Park Football Club for the season 2024/25. On signing this agreement, I am agreeing to abide by the rules issued by the Management Committee and the Codes of Conduct of the Club and to ensure the player/member named above will do so. I also agree to pay the annual membership fees promptly as requested. I accept that HPFC- and/or its representatives will not be held responsible for any accident or injury that involves my son/daughter whilst in their care.
